We are reaching out to share an important update regarding our care team at Tulsa Family Psychiatry & Wellness.

Dr. Surja will be transitioning out of our practice at the end of January 2026.

Dr. Surja has served as our Substance Use Disorder (SUD) specialist. If you were seeing him for substance use treatment, we recommend continued care with OSU Medicine’s Addiction Recovery Clinic, which offers comprehensive services and support for this level of care:

If you were seeing Dr. Surja for non-SUD psychiatric care, please know that our commitment to your treatment remains unchanged. We have a team of dedicated providers available and can assist with scheduling you with another clinician in our practice to ensure continuity of care.

Dr. Surja is also opting out of insurance and will be seeing patients on a self-pay basis in his private practice beginning in early 2026. If you wish to continue care with him, you may visit his website for additional information:

We understand that provider transitions can feel disruptive, and we want to reassure you that we are here to support you during this time. If you choose to pursue care elsewhere, we will gladly assist with transferring your medical records to another provider or facility.
